A.S.H. ADOPTION FORM AND CONDITIONS

PLEASE READ THIS PAGE COMPLETLY BEFORE CONSIDERING ADOPTING AN ANIMAL FROM ASH

When you adopt a dog from ASH, we require you to sign a form where you agree to the following conditions of adoption:

» You will accept full legal and medical responsibility for the adopted animal(s) upon taking it/them

» I understand that a home check, either before and/or after I/we take my/our dog(s) home, is standard

» I/we will keep vaccinations up to date and take my/our dog to the vet if he/she is not well.

» I/we will undertake to have my/our dog spayed, either when she is 7 months old, or after her first season or a.s.a.p. if over that age now and not done yet.

» I/we will give my/our dog(s) regular exercise and a warm, safe place to sleep.

» I/we will have my/our dog collared and tagged (it is the law)

» I/we will love my/our dog(s) and treat him/her as part of the family

» If for any reason things don’t work out, you must bring the dog(s) back to A.S.H. RESCUE – no matter how long after the adoption
